Services / Physicals
Sports - these are required in the state of Kansas if junior/senior high school students wish to participate in sports activities. Physicals are required prior to beginning any activity. Physical forms are available at the school.
Cosmetology- this is a brief physical required prior to beginning beauty school. The school supplies the physical form
Foster Parent- this physical if performed for adults who want to take care of foster children.
School- this is a brief physical required for teachers and paraprofessionals wishing to work in a school district.
Kan Be Healthy- this is the physical required for all children who receive medical cards. The physicals are done at specified intervals in the child’s development. Part of this physical is filled out by the parent or guardian prior to the child’s appointment.
Well Child- these physicals track the growth and development of children at specified intervals to make sure no problems develop as the child grows. PrairieStar has these physical forms and growth charts available.
Daycare- this physical is for the child prior to entering a daycare setting.
Pre-employment- these physicals may be required by a company prior to entry into a position within the company. Many companies require physicals and their requirements vary. Employers will provide potential employees with a physical form.
Well woman- this physical is performed for a woman on a yearly basis to check for sexually transmitted disease, cancer (breast and cervical) and general health. A pap smear, breast exam, cultures and routine office and lab tests may be done.
Department of Transportation (DOT)- this physical is a yearly requirement for the employees who drive trucks or operate heavy equipment. The physical may be done more often if a problem (such as high blood pressure or blood sugar) is identified. The employer will supply the physical form.
SRS/Disability- these physicals are required by the state to determine if an individual is able to work, or if Medicaid is necessary due to health status. The requirements for obtaining state insurance are set by the state. Yearly renewal of the physical is required in order to continue to qualify for assistance.
